Therapeutic Carbohydrate Reduction in Type 1 Diabetes: A Guide for Dietitians and Nutritionists is an innovative, evidence-driven resource designed to equip healthcare professionals with the tools to help individuals with Type 1 diabetes achieve more consistent blood glucose control.

Authored by five international experts, this guide fills the critical gap for clear, safe guidance on therapeutic carbohydrate reduction (TCR) for the management of Type 1 diabetes, providing health providers with practical strategies to support patients interested in this nutritional approach for better glycemic management.

Although carbohydrate reduction may not be suitable for every individual with Type 1 diabetes, this guide broadens the understanding and support for those who wish to personalize their treatment using TCR.

The SMHP™ position statement on therapeutic carbohydrate reduction for type 1 diabetes

This article outlines the position of the Society of Metabolic Health Practitioners (SMHP) on the use of therapeutic carbohydrate reduction (TCR) nutrition interventions for type 1 diabetes mellitus (T1DM).

The field of endocrinology has made significant strides in the treatment of T1DM, including advancements such as hybrid closed-loop insulin delivery systems, continuous glucose monitors for improved glycemic control, monoclonal antibodies to delay disease onset, and expanded access to pediatric endocrinologists. Despite these innovations, standard care for T1DM still results in excessive morbidity and mortality, with a high prevalence of microvascular and macrovascular complications. Insulin resistance, even in well-controlled T1DM, remains an independent risk factor for poor outcomes.

In 2021, only 21% of adults with T1DM in the United States achieved the American Diabetes Association's (ADA’s) target hemoglobin A1C goal of < 7.0%, with poorer glycemic control reported in pediatric and adolescent populations. Based on observational and interventional evidence, the SMHP advocates for a reevaluation of current nutritional therapies for T1DM, with a broader consideration of TCR. The SMHP recommends that TCR nutrition interventions be made more accessible and supported clinically for individuals with T1DM of all ages, and calls for increased research and attention to TCR in the management of T1DM.
